The most famous wild hot springs in Italy — sulfurous water at 37°C, tumbling into limestone pools, for free, in the open. Worth a stop if you're in the Maremma; go at dawn or night to avoid the crowds.

The thermal baths of the high Alps — the Bagni Vecchi (the Roman baths, carved in the rock, with the views) and the Bagni Nuovi (the grand spa). Worth a day for the Roman baths and the alpine setting, between Stelvio and Lake Como.

The most elegant of the Italian spa towns — the art-nouveau bathhouses, the parks, and the funicular to Montecatini Alto. Worth a day if you love Belle Époque; a night for the full resort.

Ischia
Ischia is worth two or three nights if you want thermal spas, beaches, and a slower island pace. It is the better alternative to Capri for travelers who want to stay, not day-trip.
